External Quality Initiatives
Masspro is one of the leading performance improvement organizations in the U.S. dedicated to advancing healthcare quality. Founded by the Massachusetts Medical Society which publishes the New England Journal of Medicine, Masspro transforms healthcare through developing and disseminating innovative solutions across all sections of the healthcare delivery system.
The Nursing Home Quality Initiative (NHQI) is a national effort led by the Centers for Medicare and Medicaid (CMS) to improve the quality of care for nursing home residents and provides consumers with information to help make more informed health care decisions.
Holy Trinity works closely with this group especially in the areas of restraint use and pressure ulcers. Through this organization the facility was able to send two Registered Nurses to an educational seminar on Wound Management. After completion of the National Exam, they will be Wound Care Certified, and will be credentialed to use the initials WCC after their title. This is not the norm in the nursing home industry. Holy Trinity is unique in being able to have wound specialists in house and the ability to follow wound progress without having to send them to an outside provider. In addition, we will have direct access to current best practices.
INTERACT (Interventions to Reduce Acute Care Transfers) is a quality improvement program that focuses on the management of acute change in a resident's condition. It includes clinical and educational tools and strategies for use in everyday practice in long term care facilities.
The Advancing Excellence campaign is a major initiative of the Advancing Excellence Long Term Care Collaborative. The Collaborative assists all stakeholders of long term care supports and services to achieve the highest practicable level of physical, mental, and psychosocial well-being for all individuals receiving long term care services.
The campaign works by helping nursing homes make a difference in the lives of residents and staff by focusing on the eight goals related to its mission.
There are 8 goals the collaborative oversees:
- Staff Turnover
- Consistent Assignment
- Pressure Ulcers
- Advance Care Planning
- Resident/Family Satisfaction
- Staff Satisfaction
The Campaign works to achieve its mission by:
- Helping nursing homes make a difference in the lives of residents and staff by focusing on eight goals related to its mission
- Providing support to those on the frontlines of nursing home care,
- Providing free, practical and evidence based resources to support quality improvement efforts in America's nursing homes.
- Promote open communication and transparency among families, residents, and nursing home staff.
Holy Trinity is a member of this Initiative and utilizes its tools, webinars and suggestions for the best experience for nursing home residents.
In January 2012 Holy Trinity was selected to be one of only 15 facilities to participate.
The purpose of this group is to assure that the development and implementation of the electronic transfer form is well informed by various stakeholders in central Massachusetts.
This project is garnering national attention and we are very excited to be a part of it.
Currently there is a paper Universal Transfer Form that covers all information needed in the continuum of care. This will become an electronic form at the end of 2012. It will enable hospitals, nursing homes, and other health care venues to communicate in a quicker, more efficient manner. It will be a template allowing the nursing staff to fill the information and send electronically to the hospital, similar to an email. The hospital can treat and send the resident back in a quicker timeframe using the same electronic communication to alert the home what they have done.
MeHI - Massachusetts eHealth Institute is overseeing this project along with Dr. Larry Garber of Reliant Healthcare. A learning Collaborative will determine best practices for using the universal transfer form.

Advancing Excellence Campaign-Beta Testing Site
Holy Trinity was contacted by Mass Senior Care Association and the Colorado Foundation for Medical Care to be a ‘Beta Testing Site’ for the Advancing Excellence Campaign’s newest goal on Safely Reducing Hospitalizations Tracking Tool. What does this mean for our facility? It means that all of our efforts towards quality improvement are recognized by the Quality Improvement Organization of Massachusetts and our input is valuable to them.
We have been asked to use and evaluate the tool prior to the Advancing Excellence Campaign implementing this for the nursing homes. Our suggestions and evaluations of the tool will be taken into consideration and incorporated into the new tool prior to use.
Since we have been using the Interact protocol for almost 2 years and using their tracking tool, it was pretty straight forward to use and evaluate this tool. A plus for the facility is that when we sign on for this initiative, it will be an easy transition to use!

The Massachusetts Senior Care Association represents a diverse set of organizations that deliver a broad spectrum of services to meet the needs of older adults and people with disabilities. Since its founding in 1949, Mass Senior Care’s mission has been to improve the quality and delivery of long term care services in Massachusetts through research, education and advocacy.
To fulfill its mission, the association:
- Provides advocacy on legislative, regulatory and financing issues affecting care providers, residents, and consumers.
- Collects and analyzes data relating to the delivery and financing of senior care services.
- Develops and sponsors quality improvement and continuing education programs for facility staff.
- Provides information and referral services to consumers through its toll free consumer line (800 –care-for), free consumer publications, and online at www.masseniorcare.org.
- Educates the public and policymakers about issues relating to the care of older adults and individuals with disabilities.
A nonprofit organization, Mass Senior Care is supported by member dues and governed by a Board of Directors elected by the membership.
